How should I tell a guy I like him by passing him a note?

I am planning on telling this guy I like, that I like him. I have him in my lunch period and 7th period, and I see him in the hallways. He's kinda quiet and shy, and I'm really shy, so I was planning on passing him a note in the hallway or during class to tell him I like him. What should I put in the note, and when should I pass it to him? Or is there a better way to tell him, other than to his face?

Also: I haven't known him too long, only for about a month.
He's on our school's wrestling team.
My friend introduced me to him.
